Provider Relief Fund Portal Open for Reporting Period 2

 The PRF Reporting Portal opened for Reporting Period 2 on January 1, 2022, and will remain open through March 31, 2022, at 11:59 PM ET.

Providers who received one or more General and/or Targeted PRF payments exceeding $10,000, in the aggregate, from July 1, 2020, through December 31, 2020, must report on their use of funds in Reporting Period 2.

There are three steps to reporting:

  1. Register in the Provider Relief Fund Reporting Portal
  2. Read the reporting requirements notice, The Post-Payment Notice of Reporting Requirements updated on June 11, 2021.
  3. Complete and submit your report via the Portal

 If some or all of the funds went unused, they will need to be returned within 30 days after the end of the reporting period – April 30, 2022.
